[Feedback] Creating a blank "feedback list" in the setup section will create a blank entry that cannot be modified

Since the blank list is useless and cannot be edited, Darren mentioned that he had to actually drop the list from the DB.

Steps:
0) Enable the feedback plugin if necessary.
1) Click over to the 'helpdesk setup' section and then the 'Feedback' (tab).
2) In the "Add List" box leave the 'Name' field blank and click 'Save Changes'.

Result:
Observe that a "blank" entry is created in the 'Feedback Lists' sidebar. Since it is not a clickable link it cannot be modified or deleted.

Note:
This is the same issue that has plagued the Workers and Groups tabs in the past.

Blank entries will now default to being named "(blank)". If the database is somehow empty then the text will be replaced in the list dynamically so editing is always possible.

Verified fixed in dev build 797.

If the feedback list is left nameless, then the entry is labeled "(blank)" and can be edited later. Even if you decide to leave it as the (blank) entry, it is treated just like any other list and can be sent feedback.
